  • Visit our Care Request Form -OR- Call 505.241.9321 to arrange New Care Schedule over the phone. Have your scheduler handy and days/times of care planned – this is very important because space is limited.
  • -NEXT-
    Print, fill-out and sign this form before your child’s first day of care.
    Bring this signed form and other required documentation to First Day of Care.
    Scan and email filled out form + additional docs to before your child’s first day of care.

  • Pay the one-time $50 Set-up fee by check or money order through the mail -OR- call 505.241.9321 to pay set-up fee over the phone. You may also drop-off the set-up fee along with your child’s application and supporting documents during business hours to:

    If mailing, please do not send cash. Check or money order only payable to Madeline Carlisle LLC. It is very important that we receive all of the above before your child’s first day of care.

  • Monthly fees are due by the 5th of each month. If your child starts care after the first of the month, you will receive a one-time, prorated bill for daycare services only for the start month due within five days of your child’s first day. Billing will continue on the 5th of every month. Please let care giver know if you would like to set-up automatic bill pay.

  • If your schedule is not set in stone for your child’s first month, or so, no worries, we understand and will accommodate as best we can. I am here to make this process as easy and hassle-free as possible.

  • Please call 505.241.9321 and/or email to let caregiver know of any/all changes or errors in your child’s care schedule ASAP.
